I acknowledge that I have received, in perfect condition, all products and services from Aurxio, via an independent contractor representing Aurxio Integrations & Contracting Inc., as per arrangements made with myself and Aurxio Inc.

I acknowledge these goods and services were received without any defect and without damage to my home, business, property, or injury to any persons.

I acknowledge that the placement and location the products/devices were installed was agreed upon by myself, the owner (or authorized representative) of this location. I acknowledge that all services have been completed in full and that I will not ask for a refund or chargeback unless the service does not follow Aurxio's return policy and installation guarantee.

Internal cord-concealment was completed

Concealment Acknowledgement:

I requested and agreed to the carrying out of drywall incisions, feeding of specific customer approved wires, and placement of concealment and gang plates for the purposes of internal cord-concealment. I have the authority to make these decisions in the building in which Aurxio is operating. I acknowledge that I am fully liable for the concealment. Aurxio is not responsible for any damages that may result. I acknowledge Aurxio operates in adherence to the "2025 Electrical Code" along with specific State regulations in the State which Aurxio is operating. High-voltage cables are not placed behind the drywall by Aurxio or its representatives. This completion is the responsibility of the homeowner.

Technician Technician Acknowledgement

For this commercial project, I hereby state and represent that:

  • I am experienced in and familiar with services I am performing;
  • I hold a current, valid, Driver's License and Commercial General Liability Insurance;
  • I fully understand the risks and dangers inherent in work I am completing;
  • I am voluntarily participating in services I am completing, knowing the potential conditions and factors associated with performing these services; and
  • I expressly agree to assume the entire risk of any personal injury which I might suffer as a result of my participation in carrying out services
  • I agree to the Scope of Work on this Aurxio Work Order and associated total job technician payout (including all taxes). If additional expenses are to be used for this project that are purchased out of pocket by the technician, Aurxio may reimburse in full upon receiving a copy of the receipt.

Client Client Acknowledgement

I, the Client, acknowledge that I have received, in perfect condition, all products and services from Aurxio, via an independent contractor representing Aurxio Integrations & Contracting Inc., as per all arrangements made. I acknowledge these goods were received without any defect and without damage to my home, business, property, or injury to any persons.

If applicable, I acknowledge that Aurxio has adhered to any privacy laws, or other laws, surrounding the installation of CCTV systems in residential homes. The placement and location the cameras were installed was determined by myself, the owner of this location. Aurxio will not provide any monitoring services for my cameras view them at any point.

By signing this form, I agree to hold Aurxio Integrations & Contracting Inc., its officers, owners, employees, and independent contractors representing Aurxio Inc., entirely free from any liability, including financial responsibility for injuries or damage. This acknowledgement of the receipt of goods and services is valid and clear.

Concealment Work Agreement

This Customer Work Agreement ("Agreement") is made and entered into by the undersigned parties: Aurxio Integrations & Contracting Inc. (known as the "Company") and the undersigned (known as the "Customer").

This contract identifies the request and agreement by the customer for the carrying out of drywall incisions, feeding of specific customer approved wires, and placement of concealment and gang plates for the purposes of internal cord-concealment.

Aurxio is under the impression from the customer that they are the sole owner of their building in which Aurxio is operating.

The customer is aware that they are fully liable for the concealment. Aurxio is not responsible for any damages that may result.

This contract is to signify Aurxio operates in adherence to the "2024 Canadian Electrical Code" along with specific Provincial regulations in the Province which Aurxio is operating. High-voltage cables are not placed behind the drywall by Aurxio or its representatives. This completion is the responsibility of the homeowner.

IN WITNESS WHEREOF, the parties hereto have caused this Agreement to be executed by their duly authorized representatives, effective as of the day and year first above written.
