General Service Completion Acknowledgement

I confirm that the work described in the work order has reached substantial completion and that I have had a reasonable opportunity to inspect the visible work and receive a basic demonstration where applicable. Any known incomplete item, visible damage or concern is listed below. Signing this form confirms the information reasonably visible at the time of inspection. It does not waive Aurxio's workmanship warranty, any mandatory consumer right, any latent defect, or any claim for damage that could not reasonably have been discovered during the inspection.

TV Mounting Completion Confirmation

I approved the final television location and height before drilling. I have reviewed the visible television condition, mount position and basic operation after installation. The following concerns or incomplete items, if any, are recorded below. I understand that patching, painting, electrical work and cable concealment are included only if listed in the work order.

CCTV / Security System Handover Confirmation

I approved the final device locations and have reviewed the available live views or system operation. I received the agreed account, credential and basic-use handover. I understand that I am responsible for lawful use, notices, audio-recording settings, authorized users, passwords, recording retention and ongoing maintenance after handover. Aurxio does not monitor the system unless a separate written agreement states otherwise.

In-Wall Concealment Confirmation

I have authority to approve work at the property and approved the cable-concealment route and method described in the work order. I understand that reasonable access openings may be required, hidden obstructions can require a route or price change, and patching or painting is included only if stated. Ordinary flexible power cords and extension cords will not be concealed inside a wall. This acknowledgement does not release Aurxio from responsibility for damage caused by negligent workmanship.

Operational Job-File Photos

I understand that Aurxio may take reasonable photographs of site condition, equipment, concealed work, serial numbers and completed work for quality control, safety, warranty and job documentation. These records are handled under Aurxio's Privacy Policy.

Optional Marketing Consent

Optional — I allow Aurxio to use selected photographs or video of the completed work for its portfolio, website and social media. Aurxio will not publish my name, exact address, faces, licence plates or other identifying details without separate permission. I may withdraw permission for future use by contacting [email protected].
